Youth Homeless Demonstration Program (YHDP) Case Manager

We are hiring a Youth Homeless Demonstration Program (YHDP) Case Manager at our Eau Claire, WI office located on 418 Wisconsin Street, serving Eau Claire, Jackson, Buffalo, Trempealeau, and Chippewa counties. This position is responsible for providing case management services. The YHDP Case Manager will provide case management to youth and young adults, ages 18-24, experiencing homelessness. They will also assist with housing searches and assess, refer, and provide services necessary to maintain stable housing. Applicants must be fully vaccinated against COVID-19.
Responsibilities include assessing the needs of youth and young adults ages 18-24 experiencing homelessness; developing short and long-range goals based on the needs assessment; coordinating a service plan with other organizations and volunteers; Assist in securing permanent housing.
This position requires a bachelor’s degree in Human Services or a related field or at least four years of experience in social work, counseling, or related field. Previous experience working with individuals with mental illness and programs in a case management capacity (minimum one year) required. Preference of an individual with lived experience of homelessness or housing instability as a youth or young adult. The starting salary is $19.20 per hour based on experience
