WERA Program Case Manager

Job Description:
The WERA Program Case Manager is responsible for case management services in the WDEOC/WERA service area including conducting needs assessments and formulating individual development plans; identifying and making referrals to other resources; serving as a clearinghouse for referrals to subsidized housing, serving as an advocate for clients; provision of appropriate counseling services and follow-up activities; assistance in identifying and securing permanent housing; coordinating services with agency programs and other organizations; developing and maintaining reports and files, and maintaining a high level of data quality

This position requires a bachelor’s degree in Human Services or a related field or at least four years of experience in social work, counseling, or a related field. Previous experience working with individuals with mental illness and programs in a case management capacity (minimum one year) is required. The starting wage is $19.77 per hour based on experience. We offer a complete benefits package, including:

Employees can contribute immediately. After 1 year of employment, the agency will contribute between 3 and 6 percent, dependent on the employee's contribution.
